How to Order a Free Hotel Accommodation Voucher When members fulfill the requirements for ordering a hotel accommodation voucher with valid electronic miles earned under a same account number, members or clients can fill out the "Hotel Accommodation Voucher Application Form" at any EVA Air ticketing counter (except for the airports ticket counters) during  the office hours. Please be sure to prepare photocopy of passport  when applying. Members can also issue the Hotel Accommodation Voucher by logging in to their accounts online. This accommodation voucher is valid for one year from the date on which it was issued.  Name and Membership Card Number must be exactly the same as EVA Membership Card when you make your reservation with listed hotels. This accommodation voucher cannot be transferred, changed hotel, reassigned to a different name, or converted back to electronic miles credits after this accommodation voucher has been issued. It cannot be replaced if lost or destroyed. The accommodation voucher can only be used by the member herself/ himself at the age of 18 years or older, and cannot be transferred to someone else. Each member is also subject to reservation of only one guest room per night. Please make the prior reservation through global reservation center or hotels and contact with the hotel for more details. Please present the original accommodation voucher upon check-in.  For e-voucher holders, there is no need to present voucher. A room is available for 1-2 person(s).  Please check the room rate differential with hotels directly if you need to change the room type. Extra fee shall be applied on the following days: An extra fee of NTD1,000 per room per night for Keelung Hotel, NTD800 per room per night for Taichung Hotel and NTD500 per room per night for Tainan Hotel shall be applied to  members who stay on Saturdays, Long weekend and Eves of Taiwan National Holidays. The accommodation voucher will not be acceptable for Keelung Hotel, Taichung Hotel, Tainan Hotel and Jiaosi Hotel during Chinese New Year period. (The period shall be as per the hotel's announcement. For Jiaosi Hotel, please refer to Note 8.) Taipei Hotel: An extra fee of NTD1,000 per room per night shall be applied to members who stay during the date of DEC31 and the exhibition, show fairs such as: Taipei Int'l Cycle Show, COMPUTEX Taipei Int'l Information Technology Show and SEMICON Taiwan. (The period shall be as per the TWTC's announcement) Penang Hotel: An extra fee of MYR11.60 per room per night shall be applied during the Long weekend and Special event. Surcharge MYR23.20 per room per night shall be applied to Chinese New Year period. (The period shall be as per the hotel's announcement) Jiaosi Hotel: An extra fee of NTD600 per room per night shall be applied to members who stay on Sundays. An extra fee of NTD2,400 per room per night shall be applied to members who stay on Fridays. An extra fee of NTD4,800 per room per night shall be applied to members who stay on Saturdays, Long weekend and Eves of Taiwan National Holidtys.  Breakfast is not included in the terims of the accommodation voucher. Should guest require breakfast, the fee shall be applied. Welcome fruit, daily newspaper and mineral water will be provided by the listed hotels. (This excludes Paris Hotel.) Free parking (except Paris) and free usage of well-equipped gym, sauna (except Taipei), swimming pool (except Taipei, Paris), aerobic studio (Keelung, Jiaosi only), children's playground (Taichung, Tainan and Jiaosi only), free in-room movies (Taipei only), Lohas SPA (Jiaosi only) and Sky Spring (Jiaosi only)  are available for the listed hotels.   Above requirements are subject to the availability and terms and conditions of hotels.
